Select the Membership application form link(s) appropriate to your borrower type from the above list;
please note: You will need Adobe Reader installed on your computer to read these pdf files;
print the forms(s), fill out the required information and personally submit it to the Level 2 Loans Desk of the Kensington Library (ULA and UNSW Alumni applications can also be submitted to COFA Library);
when signing for and collecting your card you MAY be required to pay a Library membership fee(s) - select your borrower type for costs. Payment can be made at the Level 2 Service desk using any of the following: Cheque, Bankcard, Mastercard, Visa or Eftpos (cash is NOT accepted) .
